Instructions:

Also, please note, that this application MUST BE POSTMARKED NO LATER THAN
FRIDAY, DECEMBER 31, 2011!!!!
Any applications received with a postmark later than that date will be
disregarded.
Grant awards for 2012 will be announced on our website on Monday, February 14, 2012.
All approved grants will be awarded at The Josephine Foundation Follow Your Dreams Gala, to be held on Friday, June 1, 2012.
